1) Standard Primary Care Services

Whether it’s a sick visit or a routine Annual Wellness Visit, Vytalize Health provides standard primary care services that have been fine-tuned to cater to the Medicare population.

Medical professionals on our team have diverse specialties, but share their commitment in providing comprehensive care that’s organized, tech-enabled and mobile.

2) Comprehensive Care Coordination

Vytalize Health will stay involved with patients throughout the process of finding whatever support is needed, and will help them navigate the spectrum of options available to them; ranging from help with basic day-to-day activities to wound care, post-operation support, and everything in between.


While Vytalize Health does not itself provide home health aides and home care, it will happily help you manage, coordinate and arrange the procurement of such services.

3) Chronic Care Management

Chronic Care Management is one of Medicare’s newest services, and it aims to support Medicare beneficiaries by providing assistance with two or more chronic conditions.

With Chronic Care Management, eligible Medicare patients receive help from a medical professional in the creation of a comprehensive, coordinated care plan. Moreover, beneficiaries receive access to a health care professional 24/7, paired with monthly communication and phone check-ups between visits.

4) Behavioral Health

With Medicare Part B, beneficiaries may benefit from mental health services and visits, which range from psychotherapy and depression screenings to medication management and preventive visits.

Under the umbrella of behavioral health, patients receive all the above in addition to diagnostic tests, an annual wellness visit, psychiatric evaluations and so much more. All these services come together and are integrated through behavioral health to provide patients with comprehensive mental care that optimizes their path to progress.

5) House calls

Vytalize Health is particularly adept at providing in-home care, and can send a nurse practitioner to your home for standard primary care services, chronic care management, behavioral health services, and much more.

Moreover, all of the listed services are coordinated with a dedicated physician, and your nurse practitioner will provide you with a complimentary tablet through which you can communicate with a health care professional 24/7.
