2014 Annual HouseCalls Summit

XLHealth Corporation provides healthcare programs and services for chronic illnesses. The company owns and operates Medicare health plans for chronic illnesses, such as diabetes and heart failure. It offers care management support services that provides support based overall health needs to Medicare beneficiaries and their physicians; and data management and analysis. The company also provides various services that include development of care plans and goal reports to help in organizing health and care; health education programs that teach self–management of health; medication monitoring and counseling, including a comprehensive review of current medications to eliminate potential confusion over multiple prescriptions and drug interactions; and tools and supplies that helps users in monitoring health and spot warning signs of complications. In addition, it offers Medicare health plans to support providers in managing their Medicare patients. XLHealth Corporation was formerly known as Diabetex Corporation and changed its name to XLHealth Corporation in February 2004. The company was founded in 1997 and is headquartered in Baltimore, Maryland. As of February 9, 2012, XLHealth Corporation operates as a subsidiary of United HealthCare Services, Inc.

The Annual HouseCalls Summit will include all of the full-time and part-time practitioners along with some of the per assessment practitioners. The 2014 HouseCalls Summit will give the HouseCalls leadership the ability to present company updates, an opportunity for attendees to earn continuing education credits, and will also give the ability for attendees to interact with colleagues and other HouseCalls staff.

HouseCalls has gone through many exciting transitions since its commencement in 2007, which has allowed the HouseCalls program to expand exponentially and provide better service. Most recently, HouseCalls crossed their 1 Million Visit mark since the beginning of their program in 2007. This is a huge accomplishment and milestone that will be celebrated at this year’s HouseCalls Summit. This milestone is especially important because it demonstrates how many lives have been touched and exemplifies the UnitedHealth Group’s motto of, “helping people live healthier lives.” This achievement was made possible by the hard work of all the individuals within the operation, but especially because of the HouseCalls Practitioners.
