The Peaks of Health app connects you with your individually tailored wellness plan from Peaks of Health. Receive daily meal planning, exercise guidance, lifestyle modification support and motivation.
As a Peaks of Health patient, you will receive the specific content of your custom wellness plan in the app. Track your progress including calories, activities, body metrics, steps, water, and sleep in an interface shared with your Peaks of Health provider. Communicate with Peaks of Health in realtime through personal journals and direct private messages.
Intuitive Design
The app is designed specifically for Peaks of Health patients and you can't use the app unless you're a Peaks of Health patient already in their care. Use the app on your iPhone or iPad. Peaks of Health uses an admin web-based portal where they can tailor the program to your needs, monitor your progress, and communicate with you directly.
